When to use this template

  • Users currently report bugs by email or chat and every report starts a twenty-question interrogation.
  • You run a beta or early-access program and want structured defect reports from testers who are not QA professionals.
  • Your public issue tracker intimidates non-technical users and you need a gentler front end that still yields usable tickets.
  • An agency or internal tool team needs a standing channel for client-reported defects with severity attached.

Common mistakes when using this form

  • Collapsing everything into one 'describe the bug' box. You will receive 'login is broken' forty times, and each one costs an engineer a round-trip conversation the structured fields would have prevented.
  • Making the environment field a dropdown of browsers you thought of. Devices and versions are endless; a free-text field captures 'Safari on my work iPad' accurately where any fixed list fails.
  • Letting users set priority instead of severity. Reporters legitimately know how badly it hurts them; only your team can know how badly it hurts the business — collect the first, decide the second yourself.

Frequently Asked Questions

What should a bug report form include?

Five essentials: steps to reproduce, expected behavior, actual behavior, environment (device, browser, OS), and severity from the user's perspective. Contact email matters more than people think — the single follow-up question 'were you logged in?' resolves an astonishing share of mystery reports.

How do I get non-technical users to write useful bug reports?

Do not teach them to write reports; design the form so ordinary answers become one. Separate fields with concrete prompts extract structure users did not know they had. 'What did you click just before it went wrong?' gets an answer where 'reproduction steps' gets a blank stare.

Should users be able to attach screenshots to bug reports?

This version does not support file uploads. Add an optional link question and invite users to share a screenshot through their usual hosting tool, checking its access permissions first. Keep the link optional so a missing screenshot does not prevent a useful bug report.

What's the difference between bug severity and priority?

Severity is impact — data loss versus cosmetic glitch. Priority is order of work, which folds in how many users are hit, revenue at stake, and workarounds. Ask reporters for severity only; a typo on your pricing page is severity-trivial and priority-urgent, and only your team can see that.
